This privacy policy explains how I use and protect any information given when you use my service.

In terms of General Data Protection Regulations, I (Sarah Nicholson) am the data controller. Please address any enquiries about this policy to me.

I am committed to ensuring that your privacy is protected. If you provide information by which you can be identified when using my service, it will only be used in accordance with this privacy statement.

The type of personal information I collect:

I currently collect and process the following information:

  • Personal identifiers, contacts and characteristics (for example, name, address and contact details)
  • Anonymised case notes
  • Anonymised supervision notes

How I get the personal information and why I have it:

Most of the personal information I process is provided to me directly by you for the following reason:

  • In order to provide an accurate, proportionate and relevant service

I also receive personal information indirectly, from the following sources in the following scenarios:

  • In case of psychological or physical health issues that affect you accessing the service I may communicate with a third party on your behalf. I will endeavour to gain express permission from you prior to making contact with a third party.

I use the information that you have given me in order to provide you with psychotherapy, counselling and relevant support.

I may share this information with relevant professionals or next of kin if permission is given to do so and only when necessary to protect your well being or for safeguarding reasons.

Under the General Data Protection Regulation (GDPR), the lawful bases I rely on for processing this information are:

(a) Your consent. You are able to remove your consent at any time. You can do this by contacting me via email: sarahjlnicholson@gmail.com.

How I store your personal information:

By corresponding with me on the telephone, via video link, email or otherwise, or by filling in forms on this website, you may give information about yourself such as name, address and phone number. I keep this information for between 6 and 12 months. If you become a client I will hold further information about you to enable me to provide a service and for legitimate business interests. Clients information will also be kept for a longer period of time, details of which can be found in the client agreement that is provided at the outset of therapy.

Your information is securely stored at my base. If clinical notes are stored on a computer they are password protected and backed up to the cloud. Paper notes/documents are kept in a locked filing cabinet.

 

Disposal of information is done by deleting any files and/or shredding paper notes and documents.

 

Your data protection rights:

Under data protection law, you have rights including:

  • Your right of access - You have the right to ask for copies of your personal information.
  • Your right to rectification - You have the right to ask me to rectify personal information you think is inaccurate. You also have the right to ask me to complete information you think is incomplete.
  • Your right to erasure - You have the right to ask me to erase your personal information in certain circumstances.
  • Your right to restriction of processing - You have the right to ask me to restrict the processing of your personal information in certain circumstances.
  • Your right to object to processing - You have the right to object to the processing of your personal information in certain -circumstances.
  • Your right to data portability - You have the right to ask that I transfer the personal information you gave me to another organisation, or to you, in certain circumstances.

You are not required to pay any charge for exercising your rights. If you make a request, I have one month to respond to you. Please contact me at the address or phone number at the top of this website if you wish to make a request.

Disclosure of your information:

Communication with you and information held about you is confidential, except when there may be a legal or ethical obligation to share information. Limits to confidentiality include criminal activity, terrorism, radicalisation, money laundering and prevention of serious harm.

Unfortunately, the transmission of information via the internet is not completely secure. Although I do my utmost to protect your personal data, I cannot guarantee the security of your data transmitted to this site; any transmission is at your own risk. Once I have received your information I use strict procedures and security features to try to prevent unauthorised access.
